Xtreme Reality VR - Liability Waiver
For the safety of all participants, a signed liability waiver is required before participating in any Xtreme Reality VR experience. Participation is limited to ages 8 and up.
Participants under 18 must have this waiver completed by a parent or legal guardian.
By signing this waiver, I acknowledge that participation in virtual reality activities involves inherent risks, including physical movement, balance, and sensory immersion. Participation in Xtreme Reality VR experiences is limited to individuals 8 years of age or older. I voluntarily assume all risks associated with participation and agree to release, waive, and hold harmless Xtreme Reality VR, its owners, staff, and affiliates from any liability, claims, or demands arising from participation.
Motion sickness - I understand that virtual reality experiences may cause temporary discomfort for some participants, including motion sickness, dizziness, nausea, eye strain, or disorientation. Xtreme Reality VR encourages participants to take breaks as needed, and I agree to immediately notify staff and stop participation if any discomfort occurs.
I confirm that the participant is physically able to participate, has no medical conditions that would be aggravated by virtual reality activity, and agrees to follow all safety instructions provided by Xtreme Reality VR staff.
I understand that failure to follow safety instructions may result in removal from the experience.
